Earlobes change naturally with age, but can also be significantly affected by genetics or external mechanical stress, such as wearing heavy jewellery for many years. Medical treatment for an earlobe can involve either restoring volume using injections or surgical correction. For patients who have lost volume and whose earrings no longer sit neatly, synthetic hyaluronic acid is used to provide the tissue with internal stability. Preparing for a safe operation

Whether you are having injections or undergoing surgery, the procedure begins with the ear and the surrounding area being cleaned and disinfected very thoroughly with chlorhexidine to ensure absolute sterility.

The treatment

Safe care at the clinic

For injections: An extremely fine needle or blunt cannula is used to gently inject the hyaluronic acid into the earlobe. For surgery: A local anaesthetic is administered to the earlobe (making the procedure painless). The surgeon then carefully removes excess tissue or the stretched, scarred skin canal. The incision is then closed using very fine, microsurgical sutures.

After treatment

Awakening and initial rest

If you are having an injection, you can go home straight away after a short observation period. If you are having surgery, the area will be covered with surgical tape to protect the incision, relieve tension in the skin and promote optimal scar healing.

It is essential that you do not touch the area with unwashed hands. After surgery, you must keep the incision dry in accordance with your surgeon’s specific instructions, and you must not wear earrings in the area whilst it is healing.

Medical check-up and final assessment

Following surgery, a follow-up appointment is scheduled approximately 7–10 days later to remove any non-absorbable stitches. A final follow-up appointment takes place once the area has fully healed to ensure that the tissue has integrated properly.

Results following treatment

When a hyaluronic acid filler is injected, the increase in volume, the smoothing of wrinkles and the results are visible immediately. It is normal for the area to feel slightly tender and for minor bruising to occur during the first 24 hours, but this subsides quickly. With surgery, the new, smaller shape is visible immediately after the procedure, but the tissue is initially swollen and bandaged. The stitches are normally removed after about a week. The final, assessable anatomical result for both methods is achieved once the skin has healed completely and all internal swelling has subsided, which usually takes 2 to 4 weeks. For surgery, the scars then continue to fade and mature over several months, up to a year.

Who can undergo the treatment?

To undergo surgery or injection treatments with us, you must be at least 18 years old, be in good general health and (in the case of injections) not be pregnant or breastfeeding. To minimise the risk of infection and ensure complication-free healing, we do not perform any procedures on tissue showing signs of ongoing inflammation, infection or disease. If, for example, you are experiencing symptoms such as swollen earlobes, abnormally warm earlobes, itchy earlobes or extremely dry earlobes (often caused by contact dermatitis from nickel or other skin conditions), this must first be investigated and treated dermatologically before any aesthetic or reconstructive procedure can be considered.

Complications and risks

As with all medical and surgical procedures, there are risks involved, which we will discuss with you in detail. With surgery, the risks include post-operative infection, bleeding, minor asymmetries and visible scarring, although our plastic surgeons make every effort to place the incisions as discreetly as possible. With filler injections, there is a risk of unevenness, temporary bruising and, in very rare cases, vascular occlusion (where the filler affects the blood supply in a vessel). What causes asymmetry (different-sized earlobes)?

It is very common for ears to be different shapes, and this is usually down purely to your genetics. The two halves of the face are rarely completely identical. However, existing asymmetry can sometimes be exacerbated by lifestyle habits, such as consistently sleeping on the same side every night or wearing heavier earrings in just one ear over a long period of time.

How does hyaluronic acid work in an earbud?

A firm hyaluronic acid filler is injected deep into the earlobe to replace the lost fat tissue and collagen that have diminished with age. This restores fullness, smooths out wrinkled skin and gives the earlobe the physical stability and substance required for earrings to sit firmly and not tilt forwards.

Can stretched earlobes caused by 'gauges' be surgically repaired?

Yes. If you have extremely stretched openings (‘stretches’) that have lost their elasticity and can no longer close up on their own, plastic surgery is required. The surgeon carefully cuts away the scarred skin inside the stretched canal and then sutures the tissue together in several layers to restore a natural shape with as discreet a scar as possible.

What determines your price for performing an ear lobe piercing?

The price is primarily based on the medical complexity of the procedure. The cost varies depending on whether it involves stitching up a small, torn piercing hole on one side, or whether extensive, bilateral reconstruction and surgical reduction of both earlobes is required. This is always assessed and priced precisely during your consultation.

Does surgery leave a scar on the earlobe?

Any surgery that involves making incisions in the skin leaves permanent scars. However, our plastic surgeons use microsurgical techniques and extremely fine sutures. Furthermore, the incisions are usually made in the ear’s natural folds or vertically straight across the lobe, which means that the scars fade, mature and become very discreet over time.

Does it hurt to have your earlobes pierced?

During surgical procedures, an effective local anaesthetic is always administered to the area, making the operation itself completely pain-free. For injection treatments, hyaluronic acid is used which already contains a local anaesthetic (lidocaine) from the manufacturer, which numbs the tissue from within during the course of the treatment and results in minimal discomfort.

When can I get new piercings after plastic surgery?

To ensure that the tissue becomes strong enough and the internal scar is allowed to heal properly, our surgeons strongly recommend that you wait at least 3 to 6 months before getting new piercings. The new piercings should not be placed directly through the scar itself, as scar tissue is tighter and weaker than intact skin, which increases the risk of the piercing stretching out again.
