A tear trough treatment is an advanced injection treatment in which a thin, customised hyaluronic acid filler is carefully injected deep beneath the muscle in the tear trough. This is the hollow that runs from the inner corner of the eye down towards the cheek (anatomically known as a tear trough deformity). By restoring volume to this area, the shadows that form are reduced, creating a smoothing effect. The first step is a consultation with one of our injection specialists. You can choose whether to book a virtual appointment or meet us in person at the clinic. During the appointment, you will receive medical information about risks, current waiting times, and which techniques and volumes are best suited to you. Medical assessment and choice of method

Your practitioner will carry out a very thorough clinical assessment of your bone structure, skin elasticity and the lymphatic system beneath the eye. We will determine whether the hollows are due to bone structure or pigmentation, and go through your health declaration.

We will check that you have not taken any blood-thinning medication in the last few days. The area will then be thoroughly cleaned and disinfected.

The treatment

Safe care at the clinic

We use a blunt-tipped needle to minimise risks. A small puncture is made in the cheek, and the needle is inserted deeply towards the bony ridge beneath the eye. The specific hyaluronic acid is injected in microscopic doses. The treatment takes around 30 minutes.

After treatment

The practitioner will assess the results. You will be asked to look in a mirror, and we will go through the upcoming healing process with you.

Self-care and convalescence guidelines

Avoid touching the area and do not wear any make-up for 12 hours. Refrain from any strenuous exercise, saunas and exposure to extreme cold for the first 48 hours to minimise the risk of swelling. It is advisable to sleep with your head elevated.

After 14 days, the filler has fully integrated. We always carefully assess toe-valley treatments during any follow-up appointment to ensure perfect tissue integration.

Results following treatment

The results of your injection treatment are visible immediately, but it is extremely important to understand the healing process. The skin under the eye is thin and tends to retain fluid easily. Immediately afterwards, it is perfectly normal for the area to be swollen and tender. Some asymmetrical swelling and minor bruising are common during the first few days. It takes up to 14 days for the hyaluronic acid to fully integrate with your tissue and for the initial fluid build-up (oedema) to subside completely.

Who can undergo the treatment?

To undergo an injection treatment, you must be over 18 years of age. The ideal candidate for this treatment has a distinct anatomical hollow beneath the eye. If your dark circles are caused solely by superficial pigmentation, or if you have large ‘pads’ of fat and loose skin under the eyes with a tendency to retain fluid (malar oedema), fillers are not suitable as they may exacerbate the swelling. In these cases, our specialists recommend surgery or skin rejuvenation treatments instead.

Complications and risks

It is important to be well-informed about the risks associated with tear trough treatments. Many people search online to find out whether fillers under the eyes are dangerous, and there are specific risks involved. Harmless and temporary side effects of tear trough treatments include swelling and bruising. More serious complications (often referred to online as ‘botched tear trough filler’, ‘failed tear trough’ or ‘tear trough gone wrong’) usually occur when unqualified practitioners inject the wrong type of product too superficially. This can lead to the Tyndall effect (where the filler appears blue through the skin), chronic swelling or tear trough filler migration. In the event of a failed tear trough filler treatment under the eyes from another clinic, our specialists can safely and quickly dissolve the material using the enzyme Hyalase. Vascular occlusion is an extremely rare but very serious risk, which underlines the importance of medical expertise.

How long do tear troughs last?

The area under the eyes has very little muscle movement and low blood flow compared to the lips. This means that the body breaks down the hyaluronic acid very slowly. The results generally last between 12 and 18 months, and for many patients, the results last for up to several years.

Do fillers help with dark circles under the eyes?

It depends entirely on the cause. If your dark circles are caused by a structural hollow that casts a shadow, a filler will brighten the area (under-eye fillers for dark circles). If the darkness is due to genetic pigmentation in the skin or superficial blood vessels, a filler will not change the skin’s colour.

What causes a failed tear trough procedure?

We are sometimes asked about cases that have been highlighted in the media and on social media. When complications such as lumps (tear trough complications) or swelling persist for a long time, this is usually because the wrong type of filler has been used, too much volume has been injected, or the product has been placed too superficially over the muscle.

What is the PRF tear trough before and after treatment?

PRF (Platelet-Rich Fibrin) is a completely natural alternative in which we use growth factors from your own blood to stimulate collagen production. The result does not provide immediate volume like a hyaluronic acid filler, but gradually improves the skin’s structure, thickness and elasticity over a couple of months.

Can under-eye bags be treated with fillers (fillers for under-eye bags)?

If you have pronounced fat pads ("bags") under your eyes, fillers are rarely the solution, as they can add more volume to an area that is already swollen. Sometimes, however, a skilled practitioner can conceal a very small bag by injecting filler into the transition area towards the cheek, but plastic surgery (lower eyelid surgery) is often required for a genuine result.

What is the Tyndall effect (Tyndall effect tear trough)?

The Tyndall effect occurs when a hyaluronic acid filler has been injected too superficially into the thin skin under the eye. The light is then reflected in the gel, giving the skin a bluish or greyish tinge. This can be easily and safely corrected by dissolving the filler with Hyalase.

Does a Tårdal treatment hurt?

Many patients feel anxious about treatment near the eye, but the fact is that the procedure is often virtually painless. We use a blunt cannula, which means only a small, superficial prick on the cheek (where the anaesthetic is injected). The cannula then glides gently under the skin without causing any pain.
