As we age, the volume of the face changes and the skin’s elasticity decreases. At the same time, an overactive muscle in the chin area can pull the corners of the mouth downwards. The result is often a so-called downturned corner of the mouth, which creates shadows and creases running from the mouth down towards the chin. These anatomical creases are known as marionette lines (or marionette lines). A medical injection treatment involves either using a muscle relaxant (botulinum toxin) to temporarily relax the muscle, or synthetic hyaluronic acid (fillers) to restore the skin’s volume, thereby smoothing out the folds and providing a subtle lift.

The first step is a consultation with one of our injection specialists. You can choose whether to book a virtual appointment or meet us in person at the clinic. Your practitioner will carry out a thorough clinical assessment of the muscle activity around your mouth and the condition of your skin. We will determine the dosage and method (muscle relaxants, fillers or both) and go through your health declaration.

We will check that you have not taken any blood-thinning medication in the last few days. The lower part of your face will then be thoroughly cleaned and disinfected with chlorhexidine.

The treatment

When administering muscle relaxants, an extremely fine needle is used to deliver 1–2 quick, virtually painless injections into the DAO muscle. For fillers, a blunt cannula is often used to gently distribute the hyaluronic acid deep beneath the marionette line. The procedure rarely takes more than 15–30 minutes.

During a filler treatment, the area is gently massaged to ensure an even result. You will be able to look in a mirror, and we will go through the upcoming healing process with you.

Avoid touching the injection sites and do not wear any make-up for the first 12 hours. If you have had fillers, avoid strenuous exercise and heat for the first 48 hours. If you have had muscle relaxants, avoid massaging the area or lying flat for the first 4 hours.

Monitoring & results

Medical check-up and final assessment

With fillers, the volume is visible immediately but takes 14 days to fully settle. With botulinum toxin, it normally takes 4 to 10 days before the medication blocks the nerve signals and the muscle relaxes. The final result is assessed at a follow-up appointment.

Results following jawline treatment

The results depend on the method chosen. With volume-restoring treatment using fillers, the effect is visible immediately, although some swelling and tenderness are normal in the very first few days. With treatment using muscle relaxants, it normally takes 4 to 10 days for the muscle to relax and the effect to be achieved. The final, anatomically integrated result for both methods is fully assessed after 14 days.

Complications and risks

As with all injections, there are medical risks. Harmless, temporary side effects include temporary swelling, a feeling of tightness and minor bruising around the mouth. A specific risk associated with muscle relaxants in this area is that the substance may spread to the wrong muscle, which can cause temporary asymmetry when you smile (however, this effect subsides completely once the medication leaves the body). Other treatments for profile balancing

For patients with a significant amount of excess skin, or where ageing has radically altered the anatomy around the mouth, injections may have limited effectiveness. In such cases, surgery may be an option. A downturned mouth corner correction (often called a corner of the mouth lift) is a minor surgical procedure in which a small triangular piece of skin above the corner of the mouth is physically removed to create a permanent lift (which can be seen in before-and-after photos of corner of the mouth lifts). Does it hurt to treat marionette lines with fillers?

The area around the mouth is sensitive, but the treatment is usually perceived as very gentle. We sometimes apply a numbing cream beforehand, and the filler itself always contains a local anaesthetic (lidocaine) which numbs the area from within during the treatment.

How long do the results last?

The effects of muscle-relaxing injections usually last for 3 to 4 months before muscle activity gradually returns. A volume treatment for marionette lines using hyaluronic acid fillers lasts considerably longer, usually between 9 and 12 months, depending on your metabolism.

What does a procedure to correct downturned mouth corners involve?

Surgery (commissuroplasty) is a surgical option in which a plastic surgeon removes a small amount of skin just above the corner of the mouth to lift the tissue.
