The first step is a consultation with one of our injection specialists. You can choose whether to book a virtual appointment or meet us in person at the clinic. Your practitioner will carry out a thorough clinical assessment of the muscle activity around your mouth and the condition of your skin. We will determine the dosage and method (muscle relaxants, fillers or both) and go through your health declaration.

Preparing for a safe operation
We will check that you have not taken any blood-thinning medication in the last few days. The lower part of your face will then be thoroughly cleaned and disinfected with chlorhexidine.

Safe care at the clinic
When administering muscle relaxants, an extremely fine needle is used to deliver 1–2 quick, virtually painless injections into the DAO muscle. For fillers, a blunt cannula is often used to gently distribute the hyaluronic acid deep beneath the marionette line. The procedure rarely takes more than 15–30 minutes.
After treatment
Awakening and initial rest
During a filler treatment, the area is gently massaged to ensure an even result. You will be able to look in a mirror, and we will go through the upcoming healing process with you.
Recovery & healing
Self-care and convalescence guidelines
Avoid touching the injection sites and do not wear any make-up for the first 12 hours. If you have had fillers, avoid strenuous exercise and heat for the first 48 hours. If you have had muscle relaxants, avoid massaging the area or lying flat for the first 4 hours.
Monitoring & results
Medical check-up and final assessment
With fillers, the volume is visible immediately but takes 14 days to fully settle. With botulinum toxin, it normally takes 4 to 10 days before the medication blocks the nerve signals and the muscle relaxes. The final result is assessed at a follow-up appointment.

Which treatment is best for cracked corners of the mouth?
It depends on the cause of the problem. If an overactive muscle (DAO) is pulling the corners of your mouth downwards, muscle-relaxing injections are the most effective way to stop this. Can you exercise away a downturned mouth?
Facial exercises, or "face yoga", lack scientific and clinical evidence to support the claim that they can bring about a permanent change in drooping corners of the mouth or significant loss of volume. Medical treatments are generally required to achieve a measurable, anatomical result.
How does a muscle-relaxing treatment for the corners of the mouth work?
Botulinum toxin is injected in very small, precise doses into the depressor anguli oris muscle, whose function is to pull the corner of the mouth downwards. When the muscle temporarily relaxes, the muscles that pull upwards take over, allowing the corner of the mouth to rest in a more neutral, horizontal position.
What are marionette lines?
Marionette lines are the vertical lines or creases that run from the corners of the mouth straight down towards the jawline. They resemble the lines on a ventriloquist’s puppet and occur when the fat pads in the mid-face lose volume and sag with age.
Does it hurt to treat marionette lines with fillers?
The area around the mouth is sensitive, but the treatment is usually perceived as very gentle. We sometimes apply a numbing cream beforehand, and the filler itself always contains a local anaesthetic (lidocaine) which numbs the area from within during the treatment.
How long do the results last?
The effects of muscle-relaxing injections usually last for 3 to 4 months before muscle activity gradually returns. A volume treatment for marionette lines using hyaluronic acid fillers lasts considerably longer, usually between 9 and 12 months, depending on your metabolism.
What does a procedure to correct downturned mouth corners involve?
Surgery (commissuroplasty) is a surgical option in which a plastic surgeon removes a small amount of skin just above the corner of the mouth to lift the tissue. This is usually considered for older patients where drooping corners of the mouth are caused by extremely loose skin and where injections no longer provide a sufficient anatomical effect.
